Health Info Consent

I agree that ConvaTec and its affiliates (“ConvaTec”) and their third-party service providers may collect, use, transfer and process personal and health information about me for the following purposes (“Purposes”): 

1. To provide support, guidance, and information about the proper use of ConvaTec products;
2. To obtain healthcare provider, healthcare facility or supplies information, for prescription verification as necessary;
3. To interact with my healthcare provider, healthcare facility or supplier of product, for prescription verification as necessary;
4. To process sample requests; and
5. To perform other administrative tasks or to comply with applicable law.

I agree that ConvaTec may contact me by phone, text message (sms), e-mail, hard copy letter, or other means of communication to confirm or obtain the above criteria. I further understand that I must designate a phone number where I may be reached to complete the sampling process. By designating a phone number and agreeing to the Health Information Consent, I provide my consent to receive calls, text messages and/or pre-recorded or artificial voice messages from ConvaTec. I understand that consent is not necessary to do business with ConvaTec and that I can opt out of communications from ConvaTec at any time.

I understand that I can unsubscribe at any time if I do not want to receive communication from ConvaTec other than to receive product samples. I understand that if I do not agree to the ConvaTec Health Information Consent I need to call ConvaTec at 1-800-422-8811 to request my product samples
